How We Plan Our All-On-X Surgeries at Alba Dental Clinic

If you have been living with failing teeth, loose dentures, or the quiet anxiety of knowing your smile is holding you back, you have probably already spent hours researching what comes next. You may have come across the term All-On-X — a full-arch implant solution that replaces an entire row of teeth using a small number of strategically placed implants — and wondered whether it could work for you.

But before a single implant is ever placed, there is a detailed, multi-stage planning process that shapes everything: where the implants sit, how your bone anatomy guides the surgical approach, what your final teeth will look like, and whether this treatment is genuinely appropriate for your situation. At Alba Dental Clinic, this planning phase is not a formality — it is the foundation on which every successful All on Implants™ case is built.

This article walks you through exactly how our team approaches All-On-X surgery planning — from the very first conversation to the moment your final arch is securely fitted. We believe that understanding the process removes the fear, sets realistic expectations, and helps you make a decision that is right for you — not just for today, but for years to come.

Why It Matters

Planning Is the Promise

🧠

Prosthetic-Driven Design

The final teeth dictate implant position — not the other way around. This principle prevents aesthetic compromises and bite problems before they start.

✨

Authentically You

Smile design accounts for face shape, lip line, skin tone, and personality — so the result looks natural, not constructed from a catalogue.

Honest Assessment

All-On-X May Not Be Right If…

Part of planning responsibly is knowing when to say no.

⚠️

Most teeth still healthy
Individual implants or restorations may be far more appropriate

⚠️

Uncontrolled systemic conditions
Uncontrolled diabetes or autoimmune conditions raise surgical risk

⚠️

Active gum disease or infection
Any infection must be fully resolved before implant placement

✓  If All-On-X isn’t right for you now, that conversation is a beginning — not a dead end.

5 Things to Remember

01

Every plan is bespoke. The “X” in All-On-X is a variable — implant count, position, and angle are determined by your anatomy, not a standard template.

02

Imaging eliminates uncertainty. CBCT scanning maps your entire jaw in 3D — bone quality, nerve paths, sinus cavities — before a single incision.

03

Aesthetic design comes before surgery. Tooth shape, colour, gum line, and facial harmony are planned digitally — so your result is previewed before fabrication.

04

A surgical guide brings precision to theatre. The digital plan is translated into a custom-fitted guide that directs every drill placement with accuracy.

05

Honest candidacy is non-negotiable. Not everyone is right for All-On-X. A thorough assessment protects outcomes and ensures the right treatment for the right person.

More Than a Procedure — A Plan Built Around You

All-On-X is not a one-size-fits-all surgery. The “X” in the name represents a variable — typically four to six implants per arch — and up to eight in more complex cases — the precise number determined by your bone anatomy, bite forces, and overall case complexity. What works beautifully for one patient may be entirely different from the plan drawn up for the next. That variability is not a weakness in the system; it is a strength, because it means every plan is tailored to the individual rather than imposed on them.

At Alba Dental Clinic, our planning approach is informed by our clinical partnership with Dental Boutique Australia — a relationship that brings over a decade of full-arch implant experience and refined aesthetic protocols into the work we do here in Kuala Lumpur. We treat the planning phase as the most important part of the entire process. The surgery itself is the execution of a plan; getting the plan right is where the real work happens.

Stage One: The Consultation and Medical Assessment

Everything begins with a conversation. Our first appointment is not about selling a treatment — it is about understanding your situation honestly. We want to know how long you have been living with the problem, what daily life looks like for you, what concerns you most, and what kind of outcome you are hoping for. That context shapes every clinical decision that follows.

Alongside that conversation, we carry out a thorough clinical and medical assessment. We review your full medical history with care, because certain systemic conditions have a direct bearing on implant safety and healing. Uncontrolled diabetes, for example, is known to interfere with the osseointegration process — the biological fusing of implant to bone — and can increase the risk of complications significantly. Similarly, uncontrolled hypertension, autoimmune conditions, and a history of radiation therapy to the jaw area all require careful consideration before any surgical planning can proceed.

We also ask about medications (including bisphosphonates, which can affect bone metabolism), smoking habits, and oral hygiene practices. None of this is about judging your lifestyle — it is about building a complete and honest picture so that the plan we draw up is both safe and realistic. If something in your history means All-On-X carries elevated risk, we will tell you that clearly and explore what alternatives might serve you better.

Stage Two: 3D CBCT Imaging and Bone Evaluation

Once the consultation and medical history review are complete, the next step is imaging. A standard dental X-ray shows us a flat, two-dimensional view of your jaw — useful, but not sufficient for full-arch implant planning. For All-On-X, we use CBCT (Cone Beam Computed Tomography) scanning, which produces a detailed three-dimensional map of your jaw anatomy, including bone height, width, density, and the precise location of anatomical structures such as the inferior alveolar nerve and the sinus cavities.

This imaging stage is where bone quality and quantity are carefully evaluated. When teeth are lost, the jawbone beneath them begins to resorb — it gradually diminishes because it is no longer receiving the stimulation that tooth roots once provided. How much bone remains, and where it is most dense, directly influences how many implants can be placed, at what angles, and whether any additional bone preparation is needed before surgery. The CBCT data removes guesswork entirely. As one clinical principle we follow consistently: every implant placement is mapped before a single incision is made.

For upper jaw cases, the positioning of the maxillary sinus is particularly important, as implants in the posterior region must be angled carefully to work within the available bone. The posterior implants in an All-On-X arch are intentionally placed at an angle — typically up to 45 degrees — which allows them to engage the denser anterior bone and maximise stability even in patients who have experienced some degree of bone loss. This angulated placement is one of the key reasons All-On-X often avoids the need for bone grafting in cases where conventional implant placement would require it.

Stage Three: Aesthetic Smile Design

This is the stage that sets Alba Dental Clinic’s approach apart from a purely surgical one. For us, All-On-X is not just about restoring function — it is about restoring confidence, expression, and the way your face looks and feels when you smile. The aesthetic planning phase is every bit as detailed as the bone assessment.

At this stage, we consider tooth shape, size, and colour in relation to your facial features — your face shape, lip line, skin tone, and the natural contours of your expression. The goal is never a “standard” set of teeth. It is a result that looks and feels authentically like you — natural-looking, proportionate, and suited to your face rather than drawn from a template. Tooth shape selection involves considering how the upper central incisors relate to the overall face form, and colour selection aims for brightness that complements your skin tone without appearing artificial or flat.

We use digital design tools to map the planned smile before anything is fabricated, which means you can review and discuss what your final arch will look like well in advance of surgery. This is also the stage at which we consider the gum line — how much of the prosthetic gum tissue will be visible when you smile, where the transition between the restoration and your natural tissue falls, and how the lip and face will be supported. Getting these details right at the design stage is far more effective than trying to adjust them after the fact.

For patients curious about how this aesthetic thinking carries through to other restorative work at our clinic, you can explore our approach to dental implants and full-arch implant restoration on our treatment pages.

Stage Four: Digital Treatment Planning and Surgical Guide Fabrication

With the CBCT data and smile design confirmed, the next step is virtual surgical planning. Using dedicated implant planning software, our team maps out the exact position, angle, and depth of each implant within a digital model of your jaw. This is where the clinical findings from the scan, the aesthetic targets from the design phase, and the prosthetic requirements of the final arch all come together into a single, cohesive plan.

Prosthetic-driven planning is the principle we follow: the final restoration — the teeth you will be wearing — dictates where the implants need to sit, not the other way around. This is an important distinction. Placing implants in the easiest anatomical position and then building a prosthesis around them can lead to compromises in aesthetics and bite mechanics. By working backwards from the desired outcome, we ensure that the implants support the prosthesis in exactly the right way.

From this digital plan, a precision surgical guide is fabricated — a custom-made device that fits over your existing anatomy in theatre and directs the drill to each planned implant site with accuracy. This guide brings the virtual plan into the physical world and is one of the key tools that allows complex full-arch surgery to be performed with a high degree of predictability. The temporary prosthesis (the provisional arch you will wear during healing) is also designed and prepared at this stage, so it is ready for the day of surgery.

Stage Five: Surgery Day — What Actually Happens

By the time surgery day arrives, the plan has been reviewed, refined, and confirmed. The surgical team knows exactly where each implant is going and why. This preparation is what allows the procedure to move efficiently and with confidence.

The surgery is carried out under local anaesthesia for your comfort, discussed and confirmed with you in advance. The procedure typically involves the removal of any remaining non-viable teeth, preparation of the bone ridge to create a stable and aesthetically clean foundation, guided osteotomy (drilling) following the surgical guide, and placement of the implants. The posterior implants are seated at their planned angles, and multi-unit abutments are connected to ensure the prosthesis can be attached at the correct trajectory.

Once the implants are in place and primary stability has been confirmed, the provisional arch — your temporary set of teeth — is fitted. This means that in many cases, patients leave the clinic on the day of surgery with a full set of fixed teeth in place, if their bone condition, bite, and overall case are suitable. This provisional restoration is not the final result; it is a functional, natural-looking bridge designed to protect the implants and allow the surrounding tissues to heal while osseointegration takes place over the following months.

Surgery on a single arch typically takes several hours. The immediate recovery period — soft tissue healing — usually spans one to two weeks, though most patients return to comfortable daily life within a few days of the procedure.

Stage Six: Healing, Review, and Your Final Arch

After surgery, a structured review schedule begins. Regular check-ins allow the team to monitor healing, check the fit and function of the provisional arch, and make any necessary adjustments to your bite or the shape of the temporary teeth. This period of wearing the provisional restoration is also useful because it gives you — and us — a real-world trial of the aesthetic result before the final arch is committed to.

Full osseointegration — the biological process by which bone cells grow around and fuse with the titanium implants — generally takes three to six months. Once this has been confirmed through clinical and radiographic assessment, the final arch is designed, fabricated, and fitted. Final arches can be made from a range of materials — including zirconia and high-quality ceramic — chosen for their durability, natural translucency, and long-term resistance to wear and staining. The implant fixtures themselves can last many years with appropriate care; the prosthetic arch is the component that may require review or replacement over the longer term.

Ongoing maintenance — including professional hygiene visits and periodic clinical reviews — forms an essential part of long-term success. The health of the soft tissue and bone around the implants requires monitoring over time, and good oral hygiene practice at home (particularly cleaning beneath the bridge with a water flosser) makes a meaningful difference to outcomes.

When All-On-X May Not Be the Right Choice

Part of planning responsibly is being honest about when All-On-X is not appropriate. This is something we take seriously at Alba Dental Clinic. A treatment that is the right answer for one person can be the wrong answer for another, and recommending a full-arch implant procedure to someone who would be better served by a different solution does not reflect the kind of care we want to provide.

All-On-X may not be suitable if:

  • You still have most of your natural teeth in good condition. All-On-X is designed to replace an entire arch, not a few missing teeth. If the majority of your teeth are healthy and restorable, individual dental implants or other restorative options may be far more appropriate.
  • Certain systemic health conditions are uncontrolled. Uncontrolled diabetes, severe autoimmune conditions, or a history of jaw radiation therapy significantly elevate surgical risk and can impair osseointegration. In these cases, it may be necessary to defer treatment until the underlying condition is better managed, or to consider non-surgical alternatives.
  • Active gum disease or oral infection is present. Any infection in the mouth must be fully treated before implants can be placed. Proceeding with implant surgery in the presence of active periodontal disease increases the risk of failure considerably.
  • You are a smoker. Smoking is a significant risk factor for implant complications and failure. It does not automatically disqualify someone from treatment, but it does form a serious part of the risk conversation, and we would not proceed without discussing this honestly.

Bone loss, in particular, is rarely a barrier on its own — the angled implant technique reduces the need for grafting, and where more support is required, bone grafting or advanced implant techniques can usually build the foundation needed to proceed. If All-On-X is not right for you at this time, that conversation at consultation is not a dead end — it is the beginning of finding out what is right. There are other implant-based solutions and restorative pathways worth exploring, and in some cases, preparatory treatments can bring a patient to a point where All-On-X becomes viable over time.

Why the Planning Phase Changes Everything

The most common cause of dissatisfaction with full-arch implant outcomes — whether it is an aesthetic result that feels wrong, a bite that is uncomfortable, or a transition line between prosthetic gum and natural tissue that shows when smiling — can almost always be traced back to insufficient planning. The surgery itself is only as good as the plan that precedes it.

At Alba Dental Clinic, we dedicate the time we do to this process because we believe that a well-planned case is a predictable case. The CBCT imaging removes anatomical uncertainty. The aesthetic design phase ensures the result looks and feels like you. The digital surgical guide brings the plan into theatre with precision. And the honest candidacy assessment means that the patients who proceed with treatment are the ones for whom it is genuinely appropriate.

This is not the fastest way to run an implant practice. But it is the right way — and for the people who have spent years feeling self-conscious about their smile, waiting a few extra appointments for a result that is truly planned for them is almost always worth it.

If you have been considering All-On-X and want to understand what the process would look like for your specific situation, the best place to start is a no-pressure consultation with our team. We are happy to walk through your imaging, explain your options clearly, and give you an honest view of what is possible — and what is not.

Planning Is the Promise

An All-On-X surgery is one of the most significant dental decisions a person can make. The outcome — functional, long-lasting, and natural-looking — is something that can genuinely change daily life. But that outcome is not determined on the day of surgery. It is determined in the weeks and appointments that come before: in the imaging, the aesthetic design, the digital planning, and the honest clinical assessment of whether this is the right treatment for you.

At Alba Dental Clinic, every All-On-X case we take on is built on that foundation. If you are ready to understand what a full-arch implant plan could look like for you, we would be glad to be part of that conversation.
